SANTE PLUS MEDICAL CENTER is a culturally based preventive care management clinic, which aims to provide high quality primary care, and preventive care while using the patients’ primary language and taking into consideration their culture, with the integration of health education, and patient self-responsibility.
VISION
SANTE PLUS MEDICAL CENTER is built upon teamwork, equal opportunity, and is committed to developing and implementing nondiscrimination programs. Our vision is to have multiple medical centers throughout the state to serve all families regardless of language barriers. We will be able to give them quick access to care with providers who speak their language and understand their medical concerns and their culture.
SANTE PLUS MEDICAL CENTER specializes in a cultural diversity centered of care for all patients by coordinating all their healthcare needs. That's why we provide a full range of primary and preventive care services in one convenient location. Our providers focus on early intervention preventing health issues, diagnosis, treatment, and care of people with health problems, promoting good health, and managing ongoing and long-term conditions. Our providers will monitor the patients’ health and medications, order your on-site tests and screenings, and coordinate their care with the required specialists.

DOOR TO DOOR TRANSPORTATION
Get door-to-doctor transportation service when you need to see your primary care physician or specialist for regularly scheduled appointments or last-minute visits. We will draw your labs for you in office and have the results in couple days with no delay in care.
TELEHEALTH SERVICES
Patient was instructed of and verbalized understanding of the following: Telehealth is the use of electronic information and communication technologies by a health care provider to deliver services to an individual when he/she is located at a different site than the provider. Laws that protect privacy and the confidentiality of medical information also apply to telehealth.
