We take great pride in our Care Management program. This is a program where our specially trained Care
Managers work one-on-one with individuals to create a customized, shared care plan to reach and sustain the best health outcomes possible, limiting the need for individuals to go to the hospital, emergency room, or
urgent care center. Our “Call Us First” philosophy allows us to guide our patients to the most appropriate
pathway to health for each health issue encountered.
